1Hepatitis C is curable — that is the key fact
Direct-acting antivirals introduced from 2014 transformed hepatitis C from a chronic, progressive liver disease into a condition curable in most people with 8 to 12 weeks of oral tablets. WHO reports cure rates above 95% with these regimens.
Cure here means sustained virological response — the virus is undetectable months after finishing treatment. Unlike suppression therapy for some other viruses, this is an endpoint, not indefinite maintenance.
That makes the pricing question unusually stark. The barrier to eliminating a curable infection is very often the cost of a finite course of tablets.
2Where the price gap comes from
When sofosbuvir launched, its US list price was widely reported at around $1,000 per tablet, or roughly $84,000 for a 12-week course. That figure became a global reference point in the debate about medicine pricing.
In parallel, the originator granted voluntary licences permitting named generic manufacturers, several based in India, to produce and supply the molecule in a large list of lower-income countries. The Medicines Patent Pool documents this licensing model and its public-health impact.
The result is not a grey market. It is an authorised parallel supply produced under licence, which is why the same molecule can legitimately cost a few hundred dollars per course in those markets.
| Regimen | US reference list price | Indian market reference | Approximate ratio |
|---|---|---|---|
| Sofosbuvir 400 mg | ~$84,000 | ~$240 | ~350× |
| Sofosbuvir + velpatasvir | ~$74,000 | ~$320 | ~230× |
| Ledipasvir + sofosbuvir | ~$94,000 | ~$280 | ~335× |
| Daclatasvir (with sofosbuvir) | ~$63,000 | ~$190 | ~330× |
3What has to happen before treatment
Hepatitis C treatment is not a self-directed purchase. Regimen and duration depend on genotype, degree of liver fibrosis or cirrhosis, previous treatment history and other medicines you take.
- Confirmed diagnosis with HCV RNA testing, not antibody testing alone.
- Genotype determination where it affects regimen choice.
- Assessment of liver fibrosis, since cirrhosis changes duration and monitoring.
- Hepatitis B screening, because reactivation is a recognised risk during therapy.
- A full drug-interaction review — these regimens interact with common medicines including some statins, acid-reducing drugs and antiepileptics.
- A plan for the post-treatment test that confirms cure.
4Routes to affordable treatment
In several countries hepatitis C treatment is now provided through public elimination programmes at no cost to the patient, following national purchasing agreements. That is often the best first avenue to explore.
Where it is not, manufacturer patient assistance programmes, hepatitis charities and liver-disease foundations frequently provide support. Speak to a hepatologist before considering any cross-border route, because supervision and post-treatment confirmation matter as much as the tablets.

Across our Hepatology anchors, 8 referenced molecules average a 87.1× price gap, 100% of them are at least 10× cheaper in India, and the widest documented comparison is Sofosbuvir at 117×. In our Hepatology anchors the gap is not merely large — it is the widest in the entire catalog by ratio, because voluntary licensing created an authorised parallel supply rather than a race between competitors.

Questions fréquentes
Yes. Modern direct-acting antiviral regimens cure more than 95% of treated people within 8 to 12 weeks, confirmed by an undetectable viral load sustained after treatment ends.
Voluntary licences granted by the patent holder allow named generic manufacturers to produce and supply the molecule in a defined list of countries. Combined with manufacturing scale and competition, that produces prices in the hundreds rather than tens of thousands.
Possibly. Many countries run hepatitis C elimination programmes providing treatment at little or no cost. Ask a hepatologist or your national liver charity about eligibility before pursuing any other route.
