1Why temperature matters more than it appears

Biologic medicines are proteins. Heat, freezing and repeated temperature cycling can denature them, reducing potency or causing aggregation that may increase immunogenicity. Crucially, a degraded protein product often looks completely normal.

That is the danger: unlike spoiled food, a heat-damaged biologic usually gives no visual warning. The only protection is process — verified shipping conditions and correct storage — rather than inspection alone.

2What proper shipping looks like

If you are having cold-chain medicine delivered, these are the minimum expectations.

  • Insulated container with validated coolant, not loose domestic ice packs.
  • A temperature indicator or data logger included in the box.
  • Transit time short enough to stay inside the validated period.
  • Delivery requiring a signature, so the parcel is not left in sun or heat.
  • Clear instructions on the outside stating that refrigerated contents are enclosed.
  • A named contact to call immediately if the shipment arrives warm.

3Arrival inspection

Do this before the parcel leaves your hands, and photograph it.

  1. 1Note the delivery time and how long the parcel may have been in transit or unattended.
  2. 2Check any temperature indicator immediately and photograph the reading.
  3. 3Confirm coolant packs are still cold and have not fully melted.

4Storage at home

Use a dedicated refrigerator thermometer and check it weekly. Domestic fridges vary considerably between shelves, and door storage is subject to large temperature swings each time the door opens.

Keep products in original cartons to protect from light, never store them in the freezer compartment or against the rear cooling plate, and observe the labelled in-use period once a pen or vial is opened. Plan for power cuts if you depend on refrigerated medicine.
